To be agreed to by participant, or participant’s parent/guardian if the participant is under the age of 18.

I understand that Messy Glow Co. owners, employees, volunteers and others may take photographs, video and/or other media recordings during class and of the class, and that these may be used for marketing and/or public relations purposes in various settings including but not limited to: Messy Glow Co. website, Instagram, Facebook, flyers, posters, and other promotional materials. I grant permission for these images and recordings to be published by Messy Glow Co. in part or in whole. I understand that if I wish to be exempt from this, I can opt out by typing ‘No’ to the Media Release on the New Member Registration Form.
